Is there water inside?
Yes, a sealed solution, but you never touch it: the pouch does not open and is not filled. You plug it in, you unplug it, that is all.
Does it stay plugged in while you use it?
No, and that is the point. You charge it for about eight minutes, take the cable off, then use it cordless.
Six hours of warmth, really?
That is the figure stated by the manufacturer. In practice it depends on the room temperature and on how you cover it: under a blanket, it lasts longer than in the open air.
Can you sleep with it?
Like any heat source, it must not stay in contact with the skin while you sleep. Slip it into the bed before you get in, then take it out.
